Many thanks for including me in your posting, will be adding more on pharmacogenetics at CorePsychBlog, as it has been my favorite subject for > 10 years, most especially regarding the comorbidity of depression and ADD, and the mixing of drugs for those problems.
My big pet peeve is 2D6 blockage by prozac and paxil, two major inhibitors with AMP [from dexedrine to adderall to vyvanse - all substrates] as interaction sites… often months down the line, but quite frequently right out of the box. -See it all the time, have been onto it for years, many aren’t looking as carefully as they should, with subsequent problems.
Then right behind that is the genetic polymorphism issue with 2D6, also not the radar sufficiently.
